What is Senior Living Link FORUM?

Senior Living Link Forum connects seniors who are being discharged from hospitals, nursing homes, skilled nursing or rehabilitation facilities, residential care homes/adult family homes, or assisted living facilities with providers of senior housing communities who have a vacancy in their establishment. Our purpose is to promote communication between service providers and social workers, case workers, and discharge planners to ensure proper and timely placement for seniors who need continued care upon discharge.

Who can post at the FORUM?

Those who can post a listing in the forum are people who are responsible for the transition and well-being of the patients who have successfully received their short term medical treatment. These individuals are at the front line of patient safety and ensure appropriate level of ongoing healthcare upon discharge. We refer to these honorable people as transitional coordinators. Transitional Coordinators include:

Discharge planners
Social workers
Case Managers
Service Providers including primary care providers, registered nurses, physical therapists, occupational therapists, and home health agencies.

What is considered a “listing” in the FORUM?

A posted “listing” contains information about an elderly person who is being discharged from their current residence. If the person requires additional care with activities of daily living (ADLs), independent activities of daily living (iADLs), medication assistance, and any other levels of services provided at various senior housing settings, then they are a candidate for being a part of our forum listings.

Who can search through the FORUM listings?

Any licensed senior housing community and long-term care facilities with:

A vacancy
The ability to provide safe and appropriate level of care for prospective Resident with dignity.
